Based upon LPAs interviews conducted Director states they will have a staff meeting to go over a new policy where staff will call for additional staff assistance when changing a diaper, if they are the only staff in the room. The policy will include that once assistance is requested via walkie or text, additional staff will arrive to assist in the classroom within the facility did not meet the above regulation as 3 staff stated they haven't been able to visually supervise all children while changing a childs diaper in the bathroom, and being the only staff present. This poses a potential health, safety or personal rights rist to children in care.. 5 minutes. Director will email LPA the staff training that notes the details of the new policy, which is signed by all staff.

Director will ensure infant, preschool and school-age programs are not commingling children at any time by reviewing policy with staff at the staff meeting and ensuring facility is sufficiently staffed. Based on interviews and records review, the facility did not abide by the license limitations specifying serving preschool children only, by commingling infants and preschool children, which poses a potential health and safety risk to children in care.
