  Director states that beginning 8/22/25 staff have begun having the bathroom sanitized per the staff handbook, using bleach and water. Based on interview and record review, the facility did not comply with the section cited above as facility was not properly disinfecting, which poses/posed a potential health, safety or personal rights risk to persons in care.
