Director stated the facility will conduct a training with staff regarding providing ouch reports to parents. Director stated they will submit to LPA a training agenda signed by all staff in attendance. Based on interviews conducted the facility did not comply with the section cited above as 2 parents stated they did not receive ouch reports when needed which poses/posed a potential health, safety or personal rights risk to persons in care.
