At 1:02 PM, LPA observed a broken facet handle in the 3 Year Old Class Per Director, will cover unoperational sink faucets with a bag and tape. Per Director, will repair both faucets. Director will submit pictures of faucets and copy of plumber invoice via email to LPA by due date of8/13/2021. room. At 1:07 PM, LPA observed one faucet to be non-operational in the S.T.E.A.M./Media room. This poses a health and safety risk to the children in care.

At 1:10 p.m., LPA observed pretzels and Director immediately discarded contaminated food items. Per Director, will conduct a Staff Training regarding proper food storage. Per Director, will submit Staff Training Agenda, notes and staff signatures to LPA via email by due date of 8/13/2021. Cheez-Its removed from their original packaging with no labeled expiration dates. At 1:13 p.m., LPA observed freezer to have 2 bags of frozen waffles, not sealed properly or labeled with expiration dates. This poses a potential health and safety risk to children in care.
