An Early Learning Program Supervisor Jessica Palacios stated that the facility already placed S3 on Administrative Leave effective 1/20/2026 until June 2026 which is S3’s retirement date and S3 will never come back to the facility again. Jessica will provide the personal rights training course to all staff again and email LPA the confirmation letter or sheet on or before 05/15/2026. Based on the self-reported unusual incident report, staff interviews, and records review, it was determined that S3 intentionally stepped on C1’s heels and the facility placed S3 on Administrative Leave for unprofessional conduct toward children which poses an immediate health, safety or personal rights risk to the persons in care.
