Director posted the lead sampling test results. The outlet was closed off with sign "Do not use". The Director stated they will retest the outlet and provide copies of the new test results to LPA by 5/10/23. Lab report analysis water from Outlet F (6.0 ppb) taken on 03/04/2023 indicated levels of lead in exceedance. This requirement has not been met Director stated, they will have an additional staff providing supervision during outdoor play. On 11/28/22 they will have a staff meeting to discuss supervsion. Director will provide a written plan of correction to licensing office by 11/16/22.
Based on interviews conducted with 4 staff and 1 child it was determined that none of the 3 staff that were supervising the children on the day of the incident observed how C1 fell and hurt themselves.
