Deficient Practice Statement Based on observation, interview, and record review, the licensee did not comply with the section cited above in which poses an immediate health, safety or personal rights risk to persons in care. LPA observed S1 and S2 who was fingerprinted but was not background checked or associated with the facility caring for children. Director stated that S1 and S2 has been working at the facility Since 9/7/21. Director stated that the director thought that the applicant fingerprint response letter was the clearance letter. S1 and S2 stated that they did not receive any kind of Documentation from guardian via mail. LPA reviewed guardian and found S1 and S2 was not Associated as of 9/29/22 , POC Due Date: 10/13/2022
