101238(a) · / Facility shall be clean and in good repair. Technical Violation- 1596.954 HSC- Facility shall have a carbon monoxide detector. Technical Violation- 101239(n) CCR- Furniture and equipment shall be free of loose, sharp, or pointed parts. Technical Violation- 101429(a)(2)(C)3 CCR- 15-minute infant safe sleep documentation. During today's visit, LPA observed the Disaster and Mass Casualty Plan, a carbon monoxide detector, and 15-minute safe sleep documentation for infants available both on Brightwheel and in paper form. LPA did not observe any furniture or equipment with sharp, loose, or pointed parts. LPA observed a different mat on the changing table however, the mat was not free of tears. LPA informed the Director that all changing tables must have new, intact mats. The Director confirmed that new mats had been ordered, and LPA observed a receipt for the order with an estimated arrival date of 10/16/25. Page 1.
