Director must submit a plan stating how the facility will ensure supervision when children are in the restrooms. Director will send plan to LPA Pringle by POC date listed. One (1) child was unknowingly left inside the bathroom alone without adult supervision. This poses an immediate risk to the health, safety, and/or personal rights of the children in care. Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected. By 09/04/2024 email LPA a written plan on how the facility will ensure no child is left outside unsupervised. Based on interviews the facility did not comply with the section cited above by not ensuring no child is left without supervision at any time. By 10/02/2024 conduct an all staff training on supervision. Email LPA the agenda for the training and sign in sheet for all staff who attended the training.

Per file review staff HEIDY DELEON VERDUZCO present today did not have working eligible clearances, was not associated to this license or any other licensed facility. Civil penalty of $100 assessed.
