101212(d)(1)(C) · Reporting Requirements (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (C)Any unusual incident or child absence that threatens the physical or emotional health or safety of any child.
Director will read and review 101212(d)(1)(C) and sign a written declaration of understanding regarding Reporting Requirements. Director will also agree to call Duty officer with 24 hours. On 2/21/24, C1 was left unsupervised in classroom the entire duration of music class and not found until classroom and teachers returned. No UIR was called in or reported to CCL within 10 days of the occurrence. This poses a potential health, safety and/or personal rights risk to the children in care.

101229(a)(1) · (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.
Center has already implemented a new “Name to Face” policy where a full walk through of the area or classroom will be conducted prior to staff and children leaving the classroom. In addition, attendance is called by calling the children’s names one by one. The Director immediately implemented a training for all staff to incorporate counting and time stamps at transitions. Proof of training was provided to LPA during the investigation. Based on five of six interviews and records reviewed, C1 was left alone in the classroom for approximately thirty minutes without supervision. C1 was found by S4 and S6 upon their return to the classroom, which poses an immediate health, safety and/or personal rights risk to children in care. A $500 CIVIL PEANLTY WAS ASSESSED
