DIR will email LPA training agenda and staff sign-in, regarding child belongings being replenished weekly and staff will acknowledge plan of operation and illness policy to ensure parents are contacted are in a timely manner. Based on interview and record review, C1 was left in soiled clothing, for more then six hours after first episode of diarrhea. Failure to notify parent 4.5 hours after the first episode of diarrhea resulted in a significant delay of providing C1 with clean clothing. This failure to provide timely notification poses a potential risk to the health, safety, and personal rights of children in care.

101229(a)(1) · (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.
Director will email LPA an updated name to face count sheet, where a full walk through section is added, requiring staff inital and time stamp at each transition. Ensuring a full walk through is conducted before transition. Based on two of two interviews and records reviewed, C1 was left alone in the class restroom for approximately twelve minutes without supervision. C1 was located when staff passed through the class. Which poses an immediate health, safety and/or personal rights risk to children in care. Director will additionally email signed LIC 9224 for all parents enrolled, notifying them of Type A citation. Due to a repeat violation, $1000 is being assesed.
