LPA conducted interviews and reviewed documents that determined... Director will complete training videos pertaining to ratios on the CDSS Training website. Also director will provide a LIC9224-receipt of licensing forms to every parent who currently has a child enrolled in the program or any parent of a child who enrolles in the program with in the next 12 months. on 12/19/24, from 1:00 PM to 1:21 PM, the school-age program had 12 children attending, but it is unclear which staff member(s) provided care and supervision. Also on 12/20/24, from 1:08PM to 1:21PM, Staff #17(aide) was the only staff documented to be at the facility providing care and supervision to 12 children.
