Based on LPA's observations, documents collected and interviews,the facility was found to be out of complaince with the aforementioned citation... On 12/06/2024, from 1:11PM to 1:48PM it... Facility will provide a signed statement by all staff. This signed document will verify all staff employed at the facility understand the aformentuoned regulation cited today(101216.3(b). Also the facility will receive two unannounced visits by a Licensed Program Analyst. is unclear who supervised 15 infants. On 10/29/2024, from 11AM to 11:45AM It is unclear who supervised 16 infants. On 10/24/2024 it is unclear who supervised the 17 infants. On 9/23/2024 from 12PM to 1PM there are 17 infants in attendance, it is unclear who supervised the 17 infants. The two unannouced visits will be conducted with in the next 120 days from today.
