101212(d) · 101212 Reporting Requirements: (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.
DUring todays visit, Director provided LPA with a completed Unisual incident report (LIC624). Deficiency cleared during todays visit. Based on firle review, observations and interviews, facility did not comply with section cited above as an unusual incident was not reported to the department within 24 hours via telephone. This pose a potential risk to children in care. 101238(a) · Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.
Deficient Practice Statement Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in LPA observed paint chippings on the walls in both the Younger Preschool and Older preschool classrooms which need to be repaired. LPA observed a drinking fountain attached to the sink inside the younger preschool classroom to be in need of repair (water does not shut off). LPA observed a broken trash can inside the older preschool restroom that needs to be replaced.which poses/posed a potential health, safety or personal rights risk to persons in care.
