Deficient Practice Statement Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in LPA observed a substance under the unlatched/no baby gate kitchen sink which was "pesticide free" rodent repellent. LPA also observed a a knife in a cubby in the activity room that was at least 4.5 feet high which poses a potential health, safety or personal rights risk to persons in care.
