Licensee will provide a written declaration and a plan on how to ensure that the dog will be inaccessible and isolated away from day care children by 9/23/2022. Licensee will enroll in a class on how to how to properly respond to injury and illnesses in day care setting. On 04/22/2022, licensee violated child’s personal rights by failing to provide proper care and supervision resulting in a child sustaining serious dog bites and puncture wounds injuries while in care. This is a Type A citation and poses an immediate risk to the children in care. The Department will schedule a Supervisory Conference to further speak with Licensee Anahit Oganesyan to ensure that this type of incident does not occur in the future.
