We will reassign all staff trainings to have line of site and safety and supervision and we will also go over it in our upcoming staff meeting scheduled for July 20, 2026, and I will send a sign in sheet of the training by the POC date to Licensing. Based on interviews and observation two children C1 and C2 were allowed to leave the classroom undetected by staff and were alone on the play ground without staff supervision for sixty four seconds which is a potential health, safety, and personal rights risk of the persons in care. Per Assistant Director, due to this incident, a name to face refresher training will occur with all staff no later than October 26, 2021. A copy of the training certificates will be provided to CCL no later than October 26, 2021. In addition, an extra name to face check (roll call) is occurring during transitions. Based on interviews and record, it was determined that C1 was left unsupervised in the breezeway of the facility for approximately three minutes.
