I pulled everyone into office and reminded of supervision and having a staff meeting on May 5, 2026 and go over playground supervison. I will Inform Licensing about meeting and send over sign in/out sheet of who attended. Based on interview, observation, and record review, C1 was playing on the play structure monkey bars when they fell off the bars into the woodchips. Staff S1 & S2 were not in close proximity, they did not have visual observation of C1 and did not witness C1 fall. S1 & S2 learned of the injury after C2 & C3 brought C1 to them which is a potential health, safety, and personal rights risk to the persons in care. After school staff training was already issued. Preschool training will be happening on the December 7th and will provide LPA a copy of the training regime. Name and face procedures were reviewed and evaulated. As well as an updated name and face sheets shall be disussed in the trainings. Based on interviews and records, it was determined that C1 was left unsupervised in the outside play yard of the facility for an approximately one minute.
