No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. The Director changed transition protocols; removed music from free time; changed time of arts/crafts and moved magnet play from floor to tables for more control and then trained staff on new protocols.
Based on observation and interviews 2 staff were present but engaged in sweeping and arts/crafts so 4 children on rug area had no visual supervision for 5 minutes or less, and magnets were thrown causing an injury near C1’s eye This poses a potential health, safety or personal rights risk to persons in care. The Director also had staff watch the Child Care Videos’ regarding care and supervision in a Child Care Center. The Director Director provided signed copies of training at the 4/7/22 visit so POC was cleared at visit.

101219(d) · Admissions Agreement 101219 (d) Modifications to the original admission agreement shall be made whenever circumstances covered in the agreement change, and shall be dated and signed by the persons specified in The director agrees to come up an admissions agreement addendum page to be reviewed and signed and kept in the child's file whenever agreement changes are agreed upon with the families. This agreement addendum was submitted at the time of visit. No further actions required.
