  101229(a)(1) · (a)...(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.

  Facility will conduct an in-service training on how to properly supervise children and methods for transitioning children from one area to another, as well as an submit agenda and sign in/out sheet for the in-service training. Based on interview and record review, including video footage, the licensee did not comply with the section cited above in that: Child 1 (C1) was left in a classroom, without visual observation, for 6 minutes, while all other children and staff were outside of the room. This poses an immediate health, safety or personal rights risk to persons in care. The above requirements will be submitted to the Department by the POC due date.
