POC: Facility will implement corrective action pursuant to section CCR 101704 for immediate cessation of outlets testing with action level exceedance until it is replaced and retested pursuant to section CCR 101705 and returns a result at or below the Acton level. Based on records review of required lead testing results on 03/03/2023, the facility had lead values of 5.5 or above on water Outlet C, 12.90 ppb and Outlet D, 19.70 ppb, which are located in the kitchen and utilized for consumption & food prep. Director/Licensee agrees to retrain staff on infants needs and services plans and agrees to submit training topics agenda and staff participation sign in sheet. Training agenda Based on records review and interview disclosure, the licensee did not comply with the section cited above in which for the first time while in care at the facility, a child was left in their diaper for approximately three hours which caused the child to sustain a diaper rash.
