Based on interview and record review, the licensee did not comply with the section cited above in that the facility did not report an incident on 7/15/2024 where a facility van damaged a fence and the door of classroom 104. This poses/posed a potential health, safety, or personal rights risk to persons in care. ...and all relvent information required for a UIR. The facility will submit this to the department by the POC due date.

Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above in that there were 3 metal panels accessible to children in the playground. When opened there was a label on the inside of the panels that stated "Danger hazardous voltage contact will cause severe injury or death." This poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 07/30/2025
