101223(a)(2) · Personal Rights The licensee shall ensure that each child is accorded the following personal rights To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Staff will be met with, trained on supervision, and coached on supporting children closely. Center Director will work on continuous ongoing observation and feedback meetings to monitoring staff supervision skills on a weekly basis. Director will also ensure a change of clothes is readily available. On or before 11/21/25, center Director will submit a plan of action to LPA Plumboy regarding training and how to ensure children in care will have extra clothes available.
Based on the self-reported incident and interview, a child in care was walking around without one shoe, wet clothes, and wet socks. The staff were unable to find the child’s other shoe and the child had fallen and due to the rain and water outdoor, his clothing were wet. When the child’s parents arrived at the center, the child in care was missing one shoe, and left by staff in wet clothes, and wet socks for at least 5 minutes which poses an immediate health and safety risk to children in care Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected. 101229(a)(1) · 101229(a)(1)Responsibility for Providing Care and Supervision:(a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.
The facility shall have staff watch the "Supervising Children in Child Care Centers" video at ccld.childcarevideos.org, obtain a written statement from each staff member outlining their understanding, and submit it to the Community Care Licensing Division by POC date. Additionally, the facility is in the process of in-house training to prevent this from happening in the future, which will be completed by 4/11/2025. Based on the interview and record review, the facility did not comly with the section cited above. Preschooler (C1) was left unattended in the playground which poses an immediate health and safety risk to children in care. The facility shall submit the agenda, plan, and staff attendance to CCLD outlining how the facility and staff will always ensure the complete supervision of all children in care.
