During delivery of complaint, the mats in the infant yard has flatten out. Director stated they will regularly inspect the mat and replace when necessary. A document stating that action will be sent to LPA by due date. LPA observed the edges of the mat were curling up and may be a tripping hazard. Based on interview, the mat was temporarily placed to prevent children from scrstching their legs on the original cushioning which was rough to touch

Director stated they will send LPA a document stating any unusual incidents will be reported to Children's Authorized Representatives and will add notes to Procare about any incidents. Based on staff interviews and record reviews, the facility did not report to Child 1's (C1) authorized representative (AR) about an unusual incident that occurred between C1 and C2 in the classroom. Report was given upon the request of AR. This poses a potential risk to health and safety of children in care. Director stated they will look into revisiting with Startwell in dealing well with children and families.

Director stated that they did a staff training on burns and reviewed burns handout with the staff. Director also stated staff were trained on heating food. Director stated June of 2022 the carpet were cleaned, staff have condcuted deep cleaning and staff wear slippers in the infant classroom. In the in service meeting staff will be reminded to clean and disinfect on a daily basis. Director stated they will also send weekly reminders to clean and disinfect their classrooms. Based on observations and interviews, staff do not wear booties inside the infant classroom, and facility was not properly disinfected in which it caused multiple cases of hand, foot, and mouth disease. This poses a potential health and safety of the children’s in care.

Director will submit the LIC624 UIR for the Hand, Foot, Mouth diseases cases in the toddler room. Based on observations and interviews Director did not report the multiple cases of hand, foot, mouth disease that occurred in the infant center to licensing office or department of public health.
