101229(a)(1) · Care and Supervision. No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time... LPA learned that a preschool child was found alone in a school age child’s bathroom and she was crying. A mandatory staff meeting to be held immediately on 08/19/2024 at the facility regarding an unusual incident. During meeting review training with all staff present regarding care and supervision of children in various areas of the facility including the bathrooms. Facility to provide a copy of the training outline and staff attendance sheet to LPA within 24 hours from inspection held on 08/19/2024. The child was in the closed bathroom for about two minutes.
