Backpack cubbies are now secured to the wall. Teacher needs to write a stament that she understands the personal rights of the children and submit it to LPA by 06/11/25.

Based on interview, the licensee did not comply with the section cited above. Child #1 couldn’t find the toy inside the cubby, so she/he put one foot onto the bottom cubby and stretched their arm to look for the toy on top of the cubby, causing the cubby to fall on top of 4 children incuding themselves.
