101223(a)(3) · To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. Director stated that she immediately addressed the diapering concerns with the Reporting Party. As a result, a new policy was established. Management staff will monitor diaper changes every hour in each classroom and mark a diapering log after each child has been checked.
C1's diaper was not changed prior to leaving the center and as result C1 left the center with a soiled diaper. This is a potential risk to the health and safety of children in care. A staff meeting was held, where staff were informed of the updated diapering policies. Copy of diaper log was provided during the inspection. The citation is cleared effective - 5/29/25.

Director stated that she immediately retrained S1 on handling different types of situations( injuries, behavioral) both in person and online. S1 received a break from teaching in the classroom to provide to shadow and observe other teachers. Director and Assistant Director have done observations
S1 was observed to yell and grab C1 while outside on the play structure after encountering a child with a bloody nose. This is risk to the health and safety of children in care. on S1 while she is teaching in the classroom. Director will provide LPA documentation outlining training for S1 by POC date- 5/26/23.

101229(a) · The licensee shall provide care and supervision as necessary to meet the children's needs. Facitly Director train all staff on supervision and making sure that the children's dietary needs are met.Training will include the allergy list and protocols. Training roster, agenda, and training documents will be submitted to CCL by POC date- 11/18/22 This requirment was not met as evidenced by C1 ingested food that was a documented allergy while at the facility. 101212(d)(1)(B) · Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.
