101223(a) · (2 101223 (a)(2) Personal Rights (a) The Licensee shall ensure that each child is accorded the following personal rights:(1) To be accorded dignity in his/her personal relationships with staff and other persons. (2)To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. A non-compliance conference will be held with Director/Administrator/Licensee ASAP. Director/Administrator/Licensee will hold an all staff meeting & view personal rights videos, discuss the importance of level of care, and submit a written action plan on their understanding of the above mentioned. This was not met as evidenced by: A staff carried an infant to a child restricted area to retrieve a baby bottle from a hot crock-pot when the infant’s leg touched the crock-pot and sustained a 2nd degree burn to the leg. Staff applied ointment not intended for initial burns to the infant’s leg. This is an immediate health and safety risk to children in care and a type “A” violation Director is to provide signatures/names of each staff members attendance. 101212(d)(1)(C) · 101212 (d) (1) (C) Reporting Requirements (d) Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. (1) Events reported shall include the following: (C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. On 7/14/2025 A infant sustained a serious burn while in care. The incident was not reported until 7/16/2025. This is a type “B” violation and health & safety risk to children in care .
