  All staff is to review title 22 regulations regarding reporting requirements and go into the next school year knowing what is appropriate to report and what is not. Submit form to LPA with signature from each staff indicating they read the requirement. Based on LPA interview and record review, the licensee did not comply with the section cited as LPA has been told of incidents that occurred within the facility that needed to have been reported to CCL which poses/posed a potential health, safety or personal rights risk to persons in care.
