The director shall ensure that proper health practices are implemented to prevent the spread of communicable diseases. This includes following exclusion policies for ill children, notifying parents, public health, and licensing of confirmed cases, and maintaining sanitation and hygiene procedures. The director will provide staff training on illness prevention protocols and submit proof of training to the Department by 9/22/25. Based on interviews and record reviews the licensee did not comply with the section cited above the licensee failed to ensure the spread of communicable disease was prevented at the facility. Interviews and record review revealed approximately 10 children contracted hand, foot, and mouth disease (HFM) while in care. which poses/posed a potential health, safety, or personal rights risk to persons in care.

The director will review reporting requirements with all administrative staff and submit written confirmation of compliance to the Department by 9/22/25. Based on interview and record review the licensee did not comply with the section cited above. The Department was not notified of this outbreak as required. which poses/posed a potential health, safety, or personal rights risk to persons in care.
