Licensee submitted the Unususal Incident Report Form to LPA on 11/03/2021. Diector and Licensee agreed to submit the UIR upon occurrence of the incident within 24 hours from the occurence of the incident Based on the interviews and record review, on 3/23/2021 incident of inappropriate interaction among day care children occurred at day care. Director stated that she prepared the incident report and provided it to LPA on 11/03/2021.Center failed to notify the department on time. This poses a potential risk to health and safety of children in care.
