As of 1/28/25, center Director implemented a new “Name to Face” policy where staff conduct counts of all children at transitions. By COB on 4/4/25, DIR will email LPA a staff sign-in and training agenda for all staff, regarding playground safety, name to face policy, and active supervision. Based on interviews and record review, it was corroborated that C1 wandered outside of the playground fencing and was left unsupervised for an unknown length of time. Which poses an immediate health, safety and or personal rights risk to children in care. Topics should include possible hazards, solutions, and zones to prevent future incident. Type A 809 Report and 809D page must be signed and provided to all staff currently employed.

By COB 4/4/25, DIR will email LPA a staff sign-in and training agenda for all staff, regarding Title 22, Section 101212: Reporting Requirements. Based on interview and record review, the licensee did not comply with the section cited above where the unusual incident on 1/15/25, involving child 1 wandering outside of the playground and being left unsupervised for an unknown length of time. Which poses a potential health, safety and or personal rights risk to children in care. DIR will additionally submit a declaration of understanding, signed by all regional management regarding UIR being submitted in a timely manner to the correct regional office, either by mail, fax or email: unusualincidentreportsD010@dss.ca.gov.
