By COB 4/18/25, Licensee will email LPA supporting documents to show that all gates on playground have been repaired and are in operating condition. Based on interview, record review and observations all nine playground gates were either chain locked or zip tied closed due to renovation and repairs. Licensee did not submit a contingency plan for approval to CCL, pending gates being repaired. This poses a potential health and safety risk to children in care. Licensee will additionally submit and updated, facility sketch, and Emergency Disaster Plan detailing evacuation routes and relocation sites.

Licensee stated that S1 left the facility on today's date (10/24/24) to initiate their background process. Licensee states that they will provide a clearance to LPA upon receiving from DOJ. Based on record review, LPA found S1 to not have a criminal background clearance. This poses an immediate health and safety or personal rights risk to children in care.
