On 01/23/23, the facility had the water filter for water outlets “I” and “K” replaced and will have them retested by 02/24/23. Based on observation, interview and record review, water outlets “I” and “K” tested over 5.5 ppb/ug L which is considered an exceedance, which poses a potential Health or Safety, or personal rights risk to persons in care.
