101238(a)(1) · Buildings and Grounds: (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.(1)The licensee shall take measures to keep the center free of flies, other insects, and rodents.
Licensee shall schedule immediate cleaning of the facility by end of day 05/28/25. The cleaning shall be conducted by a professional cleaning company that deals with rodent/rat control services for treatment and removal. The facility shall additionally continue the monthly maintenance service and keep it ongoing. Based on observations and interviews, the staff do not keep the facility free of pests as rat droppings were observed in the classrooms: #2 and #7 under the sink areas and a dead cockroach in a trap in Room #2 under the sink area, which poses a potential risk to health, saftey, and personal rights to the children in care. 101229(a) · 101229 Responsbility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs.
The facility has conducted staff meeting to review outside safety requirements and zones of supervision. The Site Director has indicated that staff will be responsible for specific areas while children play outside. The staff at the facility did not observe a child who sustained injuries while outside , which poses an immediate risk to the health, safety, and personal rights of children in care. Playground rules was reviewed with staff to highlight specific safety concerns for each area of play outside. POC has been cleared.
