Per representative we will have a staff meeting to go over the care and supervision and there will be a training on nap safety and supervision policy and provide proof of the training and sign in sheet of who attended to Licensing by the POC date. Based on interview and record review Staff walked out of the classroom and left C1 alone napping on a mat inside the classroom while the rest of the classroom was outside which is a potential health, safety, or personal rights risk to the persons in care. Per representative we will have a meeting and training on Safe Sleep practices, nap time supervision and I will provide proof of the LIC 9227 form signed by the parent and proof of training and sign in sheet by the POC date. Based on observation and record review LPAs observed Child 1 (C1) napping on their side in the infant room and there was no LIC 9227 form inside C1's file during the inspection and Safe Sleep Regulations were not being followed which is a potential health, safety, or personal rights risk to the persons in care.
