The facility has taken the following actions: Director has conducted internal investigation after placing staff on leave, and staff was later terminated. In the months leading up to the alleged incident, multiple meetings and resouces were shared with all staff regarding infant care, including feeding. Based on interviews and record review, the licensee did not comply with the section cited above as staff force fed an infant in care, which poses an potential health, safety, and/or personal rights risk to persons in care. Based on the documentation LPA obtained during today's inspection, LPA determined this may be an isolated incident involving the terminated staff. This deficiency will be been cleared at the end of today's inspection.

101229(a)(1) · 101229 Responsibility for Providing Care and Supervision (a)(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.
Per Director, the two (2) teachers responsible for providing care and supervision for the child was placed on temporarily leave, then written up for the incident. The teachers have returned on site as of today's inspection. Based on observation and interview, the licensee did not comply with the section cited above as one (1) child was left unattended in the toddler play yard, which poses an immediate health, safety, and/or personal rights risk to persons in care. During today's inspection, LPA reveiwed the Performance Improvement Plans for the teachers involved and determined it is sufficient to clear the deficiency. SUPERVISOR'S NAME: Chandra Charles
