Provider Carmen Rocha stated that she will ensure to submit the required Unusual Incident/Injury Report (UIR) LIC-624B to the San Diego Child Care Regional Office (SDCCRO) by the due date of 06/11/2024. Based on interviews conducted, licensee’s admission & record review the licensee did not comply with the above regulation, as she didn't report the 11/21/2023 incident, which resulted in an injury to C1 that required medical treatment, as required, which poses a potential health, safety, or personal rights risk to persons in care.

102391(b) · The licensee shall permit the Department to inspect the family child care home, and to privately interview children or staff, to determine compliance with or to prevent violations of family child care laws or regulations. The Department shall exercise this authority * as specified in Health and Safety Code Section 1596.8535(a).
Licensee stated that she spoke with parents and they refused for Department to interview children. Licensee impeded the LPA's attempts to interview daycare children in care, which poses an immediate threat to the health and safety of children in care.
