101229(a)(1) · 101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. The facility will partner with SSFUSD, to ensure gates are always closed during the program's operation. The facility added safety to their lesson plans to ensure the children understand the importance of staying with teachers for their safety.
The facility's self report regarding this incident as well as interview(s) conducted by LPA. This poses an immediate health, safety, or personal rights risk to children in care. The facility will also notify all the parents regarding the incident and encourage the parents to speak to their children about safety, personal responsibility, and autonomy. A copy of this notification will be sent to licensing by 03/28/25 as proof of POC. 101700.3(b)(2) · Licensees shall maintain a lead value at or below the Action Level of 5 ppb in all outlets subject to the testing requirements of these Written Directives, for the health and safety of children in care. The facility has since added a lead filter to the affected fixture (A) and retested it on 7/6/2023. Fixture A does not have any lead detected and is now in compliance with the Written Directives. The facility shall contact the lab about uploading results onto the lead sampling database and forward correspondence to LPA until results are uploaded.
Based on record review of the water sampling database, the facility was not in compliance with the above as of the initial water results uploaded on 11/23/2022. This posed a potential health, safety, or personal rights risk to persons in care The deficiency cited today shall be cleared once results are uploaded onto the lead sampling database, by set due date of 1/4/2024.
