Facility also has contacted an exterminator to install metal mesh over the weekend to seal holes through the facility and instaling traps. Based on observation and interviews, a rodent has been observed to be at facility on multiple occasions. This poses a potential health, safety or personal rights risk to children in care.

Executive director, will submitt a plan of action and schedule in maintaing the appropriate teacher to infant ratio. Based on observation and file review, it was observed that one teachere was outside pushing a stroller "buggy" with 6 infants seated inside. This poses a potential health, safety or personal rights risk to children in care.

Executive director stated that they will submit a plan on how to prevent this from happening again as well as submit documentation for a training for infant staff about infant safe sleep. Based on observations of infant classroom, 2 infants were observed sleeping on the floor with blankets placed on top of them which is a potential health and safety risk to children in care.
