Written plan- meeting notes and signing will be submitted after staff meeting Based on observation, interview, Licensee did not comply on the section cited above in that LPA observed 2 children restrained in high chairs during circle time, which poses a potential health and safety risk to children in care. Director will conduct a meeting with staff to discuss deficiency and submit meeting notes and a declration of plan moving forward to ensure supervision by 11/6/2023.

Based on interviews, the licensee did not comply with the section cited above in that Child # 1 was left outside unattended for 15-20 mins which poses/posed an immediate health, safety or personal rights risk to persons in care
