Deficient Practice Statement Based on observation, the licensee did not comply with the section cited above. During the inspection of the play yard for the school aged license, LPA Ocegueda observed that there was rusting and rubber cracking on the platform of the play structure. Also observed was a lack of cushioning material under the swing set and multiple holes and cracking on the cement area that caused potential tripping hazards. This poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 06/21/2022
