Direcotor stated stated since outbreak has implemented all staff and children 2 years and older to wear masks indoors. Director stated has conducted training on reporting requirements with all staff. Deficiency cleared at visit. staff interviews revealed staff and children 2 years and older were not wearing face coverings indoors and staff did not inform parents of COVID-19 positive cases of children and staff.
