Deficient Practice Statement Based on observation and interview, the licensee did not comply with the section cited above. During today's inspection of the kitchen, it was observed that there were cockroaches and multiple cockroach droppings in cabinets and on a wood pull out cutting board. The facility does not cook in the kitchen, however, do store the children's lunches (brought from home) and pre-packaged snacks in the kitchen. Director Davis confirmed the observation. This poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 11/17/2022

Direcotor stated stated since outbreak has implemented all staff and children 2 years and older to wear masks indoors. Director stated has conducted training on reporting requirements with all staff. Deficiency cleared at visit. staff interviews revealed staff and children 2 years and older were not wearing face coverings indoors and staff did not inform parents of COVID-19 positive cases of children and staff. This poses a potential risk to the health, safety, or personal rights of children in care.
