interview and record review Today, Site Supervisor Michelle Rojas indicated that the staff had a meeting today focusing on Supervision and Accountability Indoors and Outdoors. Site Supervisor stated new proceedure of tranisiton from outside to inside has gone into effect that all children Interviews confirmed that child #1 was left without supervision in the play yard on 11/8/2024 for about 3 minutes. This posed an immediate risk to the health, safety and/or personal rights to the children in care. A $500.00 immediate civil penalty was assessed today. will line up at the same time. Site Supervisor stated that the facility staff will walk the play yard to inspect it and ensure no children are left outside. Deficient Practice Statement Based on record review, observation and interview, the licensee did not comply with the section cited above. LPA Ocegueda observed that staff #2 who is employed and performs the duties of a Food Service Assistant was assisting a child in the restroom alone as the child's clothes got wet with water. Site Supervisor confirmed that staff #2 was employed do handle food preparation and cleaning of the kitchen and eating areas. Today, LPA observed staff #2 conduct duties related to "food service assistant" throughout the inspection. This poses a potential health, safety or personal rights risk to persons in care.
