    101238(a)(1) · 101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents.

    By POC due date 06/19/2025 Licensee shall submit a plan on how they will keep the facility sanitary and in good repair at all times. On May 20, 2025 LPA inspected classrooms for a complaint investigation and observed peeling paint in walls of classrooms 1, 4, and 5B. LPA also observed a dead fly and other insects on the windowsill of classroom 8. Classroom 2 sink was leaking water on floor making it a slipping hazard. This poses a potential health and safety risk to children in care.

    Staff removed the items immediately during the inspection and placed them in an area inaccessible to children. Deficiency corrected. On May 20, 2025 LPA inspected classrooms for a complaint investigation and observed disinfecting wipes in classroom 1 within reach of children. This poses a potential health and safety risk to children in care.     During today's inspection LPA Macias requested the following documents; Self-Certification LIC9275, Sampling Checklist Form LIC9276, Facility Sketch LIC 999, fully labeled with locations of all water outlets, and full lead report. Sample D exceedance level is 5.7. This poses a potential risk to the Health, Safety, or Personal Rights of children in care. The Director will identify the location of the lead exceedance and ensure children are unable to access the water source. The water source will be labeled and or shut off.
