Mrs. Turner has signed up staff S9 to complete a class "Teacher in Service" scheduled for October 18, 2025 that covers "challenging behaviors, practical tips for steady leadership, as well as nervous system tools for teachers to find more confidence and clarity in their role". Based on document discovered during file review, employee S9 was given a written warning discipline notice on 9/10/25 indicating that staff was witnessed to be very aggressive to some of the children. Staff was seen grabbing a child's face & pulling their arm very hard. Staff also pulled a child's head forcefully. Mrs. Turner will submit a copy of class certificate no later than 10/20/2025 to the department.

The director states that she will conduct a meeting to discuss playground transition procedures. The director states that she will send a copy of the meeting agenda, written transition procedures, and staff sign in sheet to LPA via email by 2/9/24. Based on LPA interview and review of written statements by staff, the licensee did not comply with the section cited above, in that C1 was left alone, unsupervised, outside at the facility’s playground. This poses a potential health, safety, or personal rights risk to children in care.
