The facility has terminated the staff member. POC has been cleared during today’s visit. Based on record review an incident occurred where a staff member roughly grabbing/handling two daycare children which poses a potential health, safety, or personal rights risk to persons in care. Licensee has spoken with the staff member and had the staff member written up regarding the incident. Licensee submitted documentation of the staff member’s acknowledgement on the facility’s police and Personal Rights, including the staff’s signature. Based on interviews with Licensee and one staff an incident occurred when a staff member yelled at children which poses a potential health, safety, or personal rights risk to persons in care. A Civil penalty is assessed for repeat violations, in the amount of $250 for the violation. The documentation was submitted on 01/06/2025 to the department. POC has been cleared during today’s visit.

Staff stated the facility will have all staff conduct an online training by video and submit proof of completion with staff’s signature and date to LPA by 08/27/2024. Based on interview with staff, two staff confronted each other which resulted in them yelling at each other and making inappropriate comments while children were present which poses a potential health, safety or personal rights risk to persons in care.

LPA observed Simple Site “A”, the kitchen sink’s faucet temporarily removed and currently not in use. Administrator stated the faucet will be replaced and will be flushing out the sink for 30 days before retesting. Adminstrator will notify CCL of 2nd test by email: sebastian.phouthavong@dss.ca.gov Based on record review, facility kitchen sink (Site “A”) exceeded the allowable levels of lead in the water, testing at 9.8 ppb.
