We are going to have 2 teachers in the two's room moving forward. D1 stated she will hold a staff training and provide an outline of the training to LPA Strother by 05/23/25. Based on interviews conducted with A1 and D1, adequate supervision was not provided to children, resulting in child (C1) sustaining unexplained injuries 02/20/25. Based on interviews with staff S1 and S2 there have been occasions when they were occupied changing a child’s diaper and were unable to physically aide the remaining children. One staff reported that while changing a diaper, the other staff in the room did not intervene when one child was on another child and a bite occurred, which poses a potential health, safety and/or personal rights risk(s) to children in care.

Assistant Director stated that she will work with Center Director and the District Manager to take action on repairing or replacing the equipement in disrepair and submit proof that the equipment is in safe condition by 06/01/25. This same issue was observed on the upper stairs of the same climber, was well as rough areas at the top of the spiral slide, which poses a potential health, safety and/or personal rights risk(s) to children in care. The requirement has not been met as evidenced by During the visit at 10:14am, the Director moved one child out of the 2's classroom to another classroom, bringing the 2's room within ratio. Director will submit a written procedure/plan to LPA for preventing classrooms from going above ratio by 03/12/25. Based on LPA observation at 10:10am the 2's classroom had 13 children in attendance with one teacher. D1 stated she will conduct a staff training on 07/11/24 to discuss staff self-care to avoid frustration with children, how to talk to children, using the “Tell Me What to Do Instead!” Communication is Key Handout (copy provided), and review form LIC613A Personal Rights with staff. Based on interviews conducted S1 and S3 admitted that they have yelled at day care child(ren) in care on more than one occasion, which poses a potential personal rights risk to the children in care. D1 stated S5 has additional units in ECE that were not on file but is missing one of the core courses. D1 will either submit proof that S5 has enrolled in additional ECE units that meet the requirements under the alternative teacher qualifications or submit a statement outling how S5 will be scheduled to meet ratio requirements. Based on LPA’s observation and review of transcripts on 11/16/23, Staff 5 (S5) had 6 units in ECE on file (an aide) and was providing care and supervision to a group of 12 children in the “pre-kindergarten” room without a teacher or fully qualified teacher present, which poses a potential Health and Safety risk to children in care. D1 intends to submit her written statement by 12/14/23 via email to amy.strother@dss.ca.gov Based on multiple staff interviews, staff stated that the “Two’s” classroom has operated out of ratio with 16-20 children present with a teacher and an aid that does not have ECE units on more than one occasion, which poses a potential Health and Safety risk to children in care.
