101229(a)(1) · No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This regulation was not met as evidenced by Director stated that since the incident occured, the facility schedule has been modified to reduce the number of transitions during the day and the staff responsible for supervising C1 was fired. During the visit, Director gave a copy of the updated schedule to LPA Maciel-Kashima. Based on an unusual incident report submitted to the department and Director interview, a child (C1) was left alone in a classroom for approximately 15 minutes which poses an immediate threat to the health, safety, or personal rights to children in care.
