Per representative, I will definitely get the toilets fixed in all restrooms and provide proof of the work order that the plumbing is fixed by POC date. Based on observation and interview, LPA observed one toilet inside the boy's restroom not working properly and Staff disclosed the staff restroom was clogged as of 4/13/2025, and it was not reported to Licensing which is a potential health, safety, or personal rights risk to the persons in care.
